The position of Practice Support for Elderly Care (or POH Elderly) arose because (vulnerable) elderly people with complex problems increasingly continue to live at home. This group comes to the GP for care and partly because of this, care for the elderly in GP practices is playing an increasingly important role.
The POH Elderly specializes in the problems of this patient group and has the knowledge to support this vulnerable target group. The focus areas for the POH Elderly are physical, functional, cognitive and psychological frailty. The purpose of the position is to talk to older patients at an early stage in order to provide the right care. The POH Elderly Care has a central role in this.
A POH Elderly care works within the general practice and therefore falls under the final or treatment responsibility of the general practitioner. The position focuses on support, guidance and the organization of care for older patients (75+). If necessary, the POH Elderly Care can also help with a possible referral to the social domain, home care or other specialized care providers.
